Health inequities continue to shape peoples' experiences across the health system, often driven by gaps between health, social, and community services. This panel discussion brought together service providers, clinicians, community members, and researchers to explore how collaboration across primary care, hospitals, public health, and social services can build a more equitable, person-centred system of care.

Day 2: November 25, 2025

Keynote: Jodeme Goldhar

Accelerating Integrated Care: Harnessing our Collective Potential

Across Ontario, we are seeing what becomes possible when integrated care is led through connection, trust, shared purpose, and shared priorities. Jodeme brought us together to deepen the capabilities that have enabled this progress, and to further strengthen our ability to intentionally pattern our possible future — hardwiring practices and relationships that sustain impact over time. The session explored how to shape outcomes in ways that honour both provincial priorities and the distinct needs and strengths of our local communities.

Leadership in Action: Hospital Partnerships Strengthening Integrated Care

In the face of Ontario's growing healthcare capacity crisis, hospital leaders are at the forefront of transforming care delivery through integration. Led by hospital executives, this session explored how acute care hospitals are collaborating with primary care and community partners to build a more resilient healthcare system – from improving patient outcomes to addressing the systemic pressures that affect both hospitals and the broader community. This session offered key insights on the role of hospitals in shaping the future of Ontario's healthcare system.

Guided Discussion and Summit Reflections

The Summit ended with closing sessions that considered how key takeaways from the past two days can be actioned: Personal reflections, groups discussions, and a fireside chat with leaders from the Ministry of Health, Ontario Health, and Ontario Health Teams. These events helped participants strengthen their shared understanding of system integration and shaped collective commitments to move the work forward.
