INTEGRATED CARE ACTION SUMMIT

A province-wide forum led by Ontario Health Team (OHTs), bringing together OHTs, Primary Care Networks (PCNs), and partners to advance connected care​

The Integrated Care Action Summit brings together all 58 Ontario Health Teams, Primary Care Networks, Patient Family Advisory Councils, and partners to share priorities, exchange learnings, highlight impact, and advance strategies to strengthen system integration across the continuum of care.
Celebrating Progress In Integrated Care

The first Integrated Care Action Summit took place in 2025, bringing together OHT partners from across the province for two energizing days of connection, collaboration, and action.

The Summit celebrated initiatives already making a real impact in communities across Ontario. Sessions explored topics such as population health and health equity, leading home care approaches, Health Home and Health Neighbourhood models, and PCN maturity, plus much more. Across the program, big ideas were paired with real-world, scalable examples, alongside facilitated discussions and hands-on workshops to help participants translate learnings into practical next steps.

More than a conference, the inaugural Integrated Care Action Summit showed us what is possible when partners come together – building momentum towards a more connected system of care in Ontario.
